Bulk Billing Practice Incentive Program
The Bulk Billing Practice Incentive Program (BBPIP) commenced on 1 November 2025. The program provides a quarterly incentive of 12.5% of Medicare Benefits Schedule (MBS) revenue for practices that bulk bill all Medicare-eligible patients for all eligible services.
The first BBPIP payment is scheduled for the second half of January 2026. By default, the BBPIP payment is split 50% to the practice and 50% to the general practitioner (GP). This arrangement was established despite advice from NACCHO and the broader sector that the full payment should flow to ACCHOs. While this advice was not adopted, the Department of Health, Disability and Ageing has confirmed that ACCHOs may redirect the GP’s share of the payment back to the service. As independent businesses, practices and GPs retain discretion over how BBPIP payments are managed, including which bank account receives the funds and how they are allocated afterwards. If a GP redirects their BBPIP payment to the practice, the GP does not incur an income tax liability on that amount. Practices and GPs remain responsible for determining their own tax obligations.
We have been advised that a significant number of GPs have not yet entered their bank account details in Health Professionals Online Services (HPOS). Even where an ACCHO has arranged for the GP’s BBPIP payment to be redirected to the service, each GP must still enter the relevant bank account details in HPOS to ensure the payment is received from January. One of the eligibility criteria for the BBPIP is that services must be listed as ‘Bulk Billing Only’ on the National Health Services Directory (NHSD). ACCHOs may appear under both ‘General Practice’ and ‘Indigenous Health’ categories, and billing status can differ between them. To meet program requirements, all relevant listings must display ‘Bulk Billing Only’. Currently, around half of our Member Services meet this requirement. Member Services that have not yet registered for the BBPIP can register and have their registration backdated by up to 180 days.
